    Office Hours

        North Campus and South Campus:

        • Monday – Friday: 7 am – 5 pm

        Outpatient Rehabilitation (Physical & Hand Therapy):

        • Monday – Friday: 7 am – 5:30 pm

        MRI Center:

        • Monday – Friday: 6 am – 7 pm
        • Saturday: 7 am – 6 pm

        Orthotics & Prosthetics:

        • Monday – Friday: 7:30 am – 5 pm

    Medical Emergencies

        In the event of a life-threatening medical emergency, call 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of CSOG, and they will contact us. 

    After Hours Urgent Medical Needs

        If you have an urgent orthopedic need, our Express Care clinic is open beyond normal business hours and on Saturdays from 10am to 2pm. If you have an urgent issue related to your surgery outside of regular business hours, please call our office at (719) 632-7669. A doctor is on-call 24 hours a day. Your call will be returned promptly. 

    Insurance / Payment / Billing

        At CSOG, we accept workers’ compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at (719) 632-7669 to find out if we accept your insurance plan.


        If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, and Discover.


        If we are a participating provider for your insurance company, we will bill them for any charges that are not your personal responsibility. See our financial policy for additional details.

    Referrals

        Office Visits: If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.


        Occupational/Hand Therapy: You will need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) for insurance to pay for your occupational or hand therapy treatment.


        CSOG will accept an initial referral by fax, by phone, or by email from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office.

        • Audubon Ambulatory Surgery Center 
        • Grandview Hospital 

    Prescription Requests

        Prescriptions and refills for established patients of CSOG are routinely issued during our regular office hours (see above). You can make your request by calling our office at (719) 632-7669.


        Please give our office at least 48 hours’ notice if you need a refill of your prescription during the work week. Call by noon on Friday for refills needed during the weekend.


        No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
